A non-clinical partner for the work that falls between appointments.
If you're a nurse practitioner, physician, social worker, hospice team member, or other care provider, you already know that some of what your patients need doesn't quite fit inside a clinical appointment. The processing, the emotions, the family dynamics, the complex non-medical conversations that get deferred. That's where I come in.
I'm Amanda Worthington-Wallin, a veteran emergency department registered nurse and a Master's student in Thanatology - the formal study of death, dying, and bereavement. I work as a Threshold Guide: non-clinical, trauma-informed support for people navigating grief, life transitions, and end-of-life.
I understand how healthcare systems work, how to move within them, and how to collaborate with clinical teams without overstepping scope. My role is to complement your care, not complicate it.
Referrals are appropriate when a patient or family member needs:
Space to process a diagnosis, prognosis or loss
Support with advance care planning or values clarification
Emotional and practical guidance through end-of-life
Grief support - anticipatory, acute, or long term
Help navigating a major life transition
I also support families and caregivers, not only the identified patient.
Please note: I am not able to provide crisis intervention for acute mental health conditions, active suicidal ideation or self-harm, or unstable psychiatric conditions requiring clinical management. For those situations, please refer to an appropriate mental health service.
There is no formal referral process. Simply share my contact information with your patient and let them reach out when they're ready. If you'd prefer to make a direct introduction or have questions about whether I'm an appropriate fit for a specific situation, you're welcome to contact me directly.

I'm based in Nakusp, BC and work in person throughout the West Kootenays, Kootenay Boundary, and Okanagan. Virtually, I can support patients anywhere in Canada.

I live and work on the unceded territory of the Syilx and Sinixt nations, whose relationship with this land stretches back long before wellness was an industry.
